MAJOR SPOILERS because my whole film experience can be described by my reaction to the ending. The whole damn movie, I was thinking the fantasy was real. Then, literally the last 60 seconds, dead serious, I started to think - wait.... this was all inside her head. Then she died, & I cried through the credits........... Now... The brilliance of this ending (and Del Toro's writing) is that it is open to interpretation. A reddit thread taught me that whether you believe in the Fantasy or Reality path - you can find substantial evidence to support both.
